What's the one document I can't leave without?

The trial balance as of your cutover date. Every other report can be rebuilt from bank statements; the trial balance is the only one that carries your prior years' equity, loans and fixed assets forward.

How much notice should I give?

Two weeks is normal and reasonable. Ask for the documents before you cancel the engagement, not after.
